Wednesday night is casual attire. Thursday night’s dinner is jacket/tie for men and cocktail attire for women.


For a refund, written notice of cancellation must be received by October 5, 2015 and emailed to Phoenix Medical Device Conference. Any cancellations after the deadline must be sent via email for special consideration. There is no guarantee that a refund will be issued after the cancellation deadline.
