For room reservations, please call the Montage Deer Valley at (888) 604-1301 and reference the 2014 Phoenix Medical Device Conference. Your conference rate is $340.00 and is guaranteed until August 15, 2014. Reservation requests made after the cut-off date will be confirmed based on space and rate availability.

Dress:

Wednesday night is casual attire. Thursday night’s dinner: jacket/tie for men, and cocktail attire for women preferred.

Cancelations:

For a refund, cancellations must be processed through the event website using your confirmation code no later than August 27, 2014. Any cancellations after the deadline must be sent via email to ningram@wsgr.com for special consideration. There is no guarantee that a refund will be issued after the cancellation deadline.

Attendee Bios:

We request that attendees (Steering Committee, Speakers, Sponsors, Organizers, and General Attendees) submit a biography using our online form. All biographies must be received by Friday, August 29. Click here to submit your biography.
